Should a client such as a disease management company or an insurance company prefer to continue to work with the individual patient directly, we can provide two levels of triage service. As soon as a health situation is verified, Advanced TeleHealth Solutions will hand over the monitoring analysis to the insurance case manager for further communication with the patient.
Level I Triage
Alerts always trigger a call from the clinician and a retest by the patient. The patient may require additional education about the operation of the equipment. When the retest information is confirmed as an ‘alert’ triage information will be forwarded in a timely manner to the appropriate person maintaining clinical responsibility for the patient.
Level II & III Triage
Level II & III Triage are customized based on the program and need of the client in terms of wellness, triage and chronic care management.
